Full-time & Per-Diem - Medicare Home Health RN – (Medicare/OASIS Required)
About The Position
Activa Home Health, an AccordCare Company, is seeking skilled and compassionate Registered Nurses to provide Medicare home health visits in full-time and per-diem roles across communities in Indian River, St. Lucie, and Martin Counties. Nurses deliver high-quality, patient-centered care in patients’ homes. The agency supports its team with leadership, streamlined processes, collaboration, and opportunities for professional growth and work-life balance. The role includes delivering skilled nursing care according to the Plan of Care, completing and documenting OASIS assessments in the EMR, and coordinating with the interdisciplinary care team. Clinical services include medication administration, wound care, infusion therapy, catheter care, and safe patient transferring and positioning. The role requires screening through the Care Provider Background Screening Clearinghouse.
Requirements
- Active Florida Registered Nurse (RN) license.
- Medicare home health experience.
- OASIS experience.
- Wound care experience.
- Minimum 1 year of home health experience.
- Proficiency with EMR systems.
- Strong communication, teamwork, and patient-focused skills.
- Complete screening through the Care Provider Background Screening Clearinghouse.

- WellSky Kinnser experience is preferred.
Responsibilities
- Deliver skilled nursing care according to the Plan of Care during Medicare home health visits.
- Accurately document visits in the EMR, including OASIS assessments.
- Promptly communicate changes in patient condition to the interdisciplinary care team.
- Provide medication administration, wound care, infusion therapy, catheter care, and safe patient transferring and positioning within scope of practice.
- Collaborate with Nurse Supervisors and Managers of Clinical Practice.
- Demonstrate professionalism, empathy, attention to detail, and strong clinical judgment.
