About The Position

TRU Healthcare is seeking a per-diem Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes. The role supports patients in remaining safe, comfortable, and independent through care delivered according to established care plans and physician orders. This is a flexible, per-visit position in which the nurse chooses how much to work based on availability and patient needs. Compensation is $45 per visit and $35 per hour for meetings and trainings. Mileage reimbursement is provided for travel between patient visits in the Twin Cities area.

Requirements

Current, valid Minnesota LPN license.
CPR/BLS certification.
Strong clinical, communication, and organizational skills.
Ability to work independently while collaborating effectively with the care team.
Ability to travel throughout the Twin Cities area.
Reliable transportation and willingness to travel between patient visits.
Genuine desire to make a difference in the lives of patients and their families.

Previous home health experience.

Responsibilities

Provide skilled nursing services in patients’ homes according to established care plans and physician orders.
Monitor patient conditions, identify changes in health status, and communicate concerns to appropriate care-team members.
Administer treatments as ordered and within the scope of practice.
Perform nursing assessments and provide appropriate interventions based on patient needs.
Educate patients and caregivers about medications, treatments, disease management, safety, and other aspects of care.
Document patient care accurately and promptly, and communicate with the interdisciplinary team.
Build meaningful relationships with patients and families while providing compassionate, patient-centered care.
Travel between patient visits throughout the Twin Cities area.
