About The Position

As an RN Case Manager with Home Health Care, Inc., you will take ownership of a patient caseload and help guide patients from their initial home health assessment through recovery, stabilization, and eventual discharge. You will be responsible for coordinating and overseeing an assigned home health caseload, which includes managing patients, completing OASIS assessments, performing comprehensive nursing assessments, developing and updating individualized plans of care, providing skilled nursing visits, monitoring patient progress, recognizing changes in condition and intervening appropriately, providing medication management and patient education, performing wound care and other skilled nursing services, communicating with physicians and interdisciplinary team members, educating patients and caregivers, helping prevent avoidable emergency department visits and rehospitalizations, and completing timely documentation using Homecare Homebase (HCHB). This role requires a nurse who can see beyond today's visit, connecting the dots to ensure the patient is progressing toward their goals, identifying barriers to improvement, communicating with physicians and therapy, ensuring the family understands the plan, and confirming patient safety at home. You will be the clinical quarterback of the case, bringing information together for everyone involved in the patient's care.

Requirements

  • Active Minnesota Registered Nurse (RN) license
  • Three or more years of RN experience
  • Strong comprehensive assessment skills
  • Excellent clinical judgment and critical thinking
  • Strong organization and time-management abilities
  • Confidence managing multiple patients and priorities
  • Excellent physician, patient, caregiver, and interdisciplinary communication
  • Ability to work independently in a field-based environment
  • Reliable transportation for patient visits throughout your assigned territory

Nice To Haves

  • Home health experience preferred
  • OASIS experience highly valuable
  • Experienced nurses from hospital, acute care, rehabilitation, skilled nursing, and other clinical environments are also encouraged to connect with us.

Responsibilities

  • Managing an assigned home health patient caseload
  • Completing OASIS Start of Care, Recertification, and Discharge assessments
  • Performing comprehensive nursing assessments
  • Developing and updating individualized plans of care
  • Providing skilled nursing visits
  • Monitoring patient progress throughout the episode of care

  • Providing medication management and patient education
  • Performing wound care and other skilled nursing services

  • Coordinating with PT, OT, SLP, social work, and other interdisciplinary team members
  • Educating patients and caregivers throughout recovery
  • Helping prevent avoidable emergency department visits and rehospitalizations
  • Completing timely documentation using Homecare Homebase (HCHB)
