About The Position
Community Care, Inc. is seeking a compassionate and dedicated Registered Nurse to provide home health care to clients in the Westmoreland and Fayette County area. The role involves building meaningful connections, delivering personalized care, and working in a supportive home setting. The RN conducts skilled nursing visits in patients’ homes, develops and implements individualized care plans, educates patients and caregivers, collaborates with a multidisciplinary team, and documents visits and patient progress. The position is full-time, day shift, Monday through Friday, 8:00 AM to 4:00 PM, based in New Stanton, Pennsylvania.
Requirements
- Current Registered Nurse (RN) license in Pennsylvania.
- Graduate of an accredited nursing program.
- Minimum of 1 year of clinical nursing experience.
- CPR certification required.
- Reliable transportation and a valid driver’s license.
- A caring heart, strong independence, and a passion for pediatric care.
Nice To Haves
- Home Health experience preferred.
Responsibilities
- Conduct skilled nursing visits in patients’ homes, including assessment, reassessment, medication administration, IV therapy, and treatments.
- Develop and implement individualized care plans.
- Educate patients and caregivers on managing conditions and medications.
- Collaborate with a multidisciplinary team to ensure continuity of care.
- Document visits and patient progress accurately and timely.
