About The Position
Community Care, Inc. is hiring a compassionate Registered Nurse to provide home health care to clients in the Westmoreland and Fayette County area. The role involves skilled nursing visits in patients’ homes, individualized care planning, patient and caregiver education, and collaboration with a multidisciplinary team to support continuity of care. The position is full-time, day shift, based in New Stanton, Pennsylvania, with a Monday–Friday schedule from 8:00 AM to 4:00 PM.
Requirements
- Current Registered Nurse (RN) license in Pennsylvania.
- Graduate of an accredited nursing program.
- At least 1 year of clinical nursing experience.
- CPR certification.
- Reliable transportation and a valid driver’s license.
- Caring, independent approach and a passion for pediatric care.
Nice To Haves
- Home health experience preferred.
Responsibilities
- Conduct skilled nursing visits in patients’ homes, including assessments, reassessments, medication administration, IV therapy, and treatments.
- Develop and implement individualized care plans.
- Educate patients and caregivers on managing conditions and medications.
- Collaborate with a multidisciplinary team to ensure continuity of care.
- Document visits and patient progress accurately and promptly.
