About The Position
The most experienced nurses know that great patient care isn't just about carrying out a treatment plan. It's about recognizing subtle changes, educating families, preventing complications, and making clinical decisions that improve outcomes. As an RN Case Manager, you'll practice at the top of your license while serving as the clinical leader for your patients throughout the St. Paul area. This is a role where your knowledge, experience, and judgment genuinely shape the course of a patient's recovery. As a Case Manager, you're not simply responding to patient needs. You're anticipating them. You'll oversee each patient's episode of care, coordinate services across multiple disciplines, monitor progress, educate caregivers, and identify potential concerns before they become larger problems. Your clinical expertise becomes the foundation for safer recoveries and better patient outcomes.
Requirements
- Active Minnesota Registered Nurse (RN) license
- Three or more years of RN experience
- Excellent clinical assessment and critical thinking skills
- Strong communication and leadership abilities
- Comfortable managing an independent schedule
- Reliable transportation for travel throughout the St. Paul territory
Nice To Haves
- Home health experience preferred
Responsibilities
- Manage a home health patient caseload throughout the St. Paul service area
- Complete OASIS assessments and develop individualized care plans
- Perform skilled nursing visits and comprehensive clinical evaluations
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals
- Provide medication management, disease education, and ongoing patient support
- Monitor patient progress and adjust care plans as needed
- Complete documentation using Homecare Homebase (HCHB)
- Help patients remain safely at home while reducing avoidable hospitalizations
