About The Position

This is a travel RN role with a focus on direct patient care, assessment, and coordinating care in a geriatric long-term care environment. The position is in a hospital-based long-term care setting with geriatric residents, and wound care experience is welcome. No tracheostomies are required.

Requirements

• NY State Registered Nurse license (active, in good standing)
• Basic Life Support (BLS) certification from the American Heart Association
• Minimum 2 years of experience in long-term care; LTAC experience preferred
• Experience with wound care in a geriatric population
• Ability to work in a long-term care environment with 12-hour shifts; willingness to cover days/evenings/nights, weekends, on-call and holidays per unit need

Nice To Haves

• Telemetry experience may be beneficial

Responsibilities

• Provide direct nursing care to geriatric residents in a long-term care setting
• Conduct comprehensive assessments, document findings, and implement the nursing plan of care
• Administer medications and monitor responses; manage wound care per facility standards
• Oversee and delegate tasks to LPNs and CNAs; coordinate care with the interdisciplinary team
• Monitor residents for changes in condition; respond quickly to clinical needs and document accurately
• Communicate clearly with families, team members, and other health professionals; maintain thorough and timely documentation
