Registered Nurse / Licensed Practical Nurse (RN/LPN)
About The Position
Elite Care Management is seeking a Registered Nurse or Licensed Practical Nurse for a part-time home health care position serving a patient in Sleepy Hollow. The role provides one-on-one care to a single patient for the full shift in the comfort of the patient's home. The nurse will get to know the patient and family, understand their in-home needs, and plan, organize, and assist with home health care services. The position also involves assisting with activities of daily living, ensuring patient comfort, and supporting overall well-being and quality of life. The listed shifts are 10 a.m. to 10 p.m. Wednesday through Saturday. PRN availability is also mentioned. The employer offers paid orientation and patient training, weekly pay by direct deposit, 24/7 on-call support, and a hands-on training lab.
Requirements
- Current RN or LPN state license.
- Current CPR card.
- Driver's license.
- Social Security card.
- Auto insurance card.
- TB test results: a two-step test or chest X-ray within the last 12 months, or QuantiFERON Gold blood work within the last 5 years.
Responsibilities
- Observe the patient, record significant conditions and reactions, and notify a supervisor or physician of the patient's condition, reactions to drugs or treatments, and significant incidents.
- Demonstrate understanding of organizational and nursing policies and procedures.
- Communicate effectively, both verbally and in writing.
- Manage and direct patient care according to the patient care plan and update the plan as needed.
- Administer prescribed medications and treatments, and monitor and document the effectiveness of interventions.
- Respond as appropriate to changes in the patient's condition and report changes to the appropriate physician or supervisor.
- Assist the patient with daily living activities, comfort, and overall well-being.
