About The Position

Elite Care Management is hiring a Registered Nurse or Licensed Practical Nurse for one-on-one home health care in a patient's home in Sleepy Hollow. The nurse works with one patient for the entire shift, gets to know the patient and family, and plans, organizes, and assists with home health care services. The role includes supporting daily living activities, comfort, and overall well-being to enhance the patient's quality of life. Part-time shifts are 10 a.m. to 10 p.m. on Wednesday, Thursday, Friday, and Saturday. PRN work is also available.

Requirements

Current RN or LPN state license.
Current CPR card.
Driver's license.
Social Security card.
Auto insurance card.
TB test results: a two-step test or chest X-ray within the last 12 months, or QuantiFERON Gold blood work within the last 5 years.
Strong spoken and written communication skills.
Ability to manage and direct care according to a patient care plan.

Responsibilities

Observe patients, record significant conditions and reactions, and notify a supervisor or physician about the patient's condition, response to drugs or treatments, and significant incidents.
Follow organizational and nursing policies and procedures.
Communicate effectively in spoken and written forms.
Manage and direct patient care according to the patient care plan and update the plan as needed.
Administer prescribed medications and treatments, and monitor and document their effectiveness.
Respond as appropriate to changes in a patient's condition and report changes to the appropriate physician or supervisor.
Assist patients with daily living activities and support their comfort and overall well-being.
