About The Position

21st Century Home Health Services (21HHS) is hiring a full-time Registered Nurse I Case Manager to provide home health care in Alameda County. The role requires field work and visits to patients in their homes. The anticipated territory includes multiple coverage areas, with a primary location assigned; occasional coverage outside that area may be required to support vacancies, leaves of absence, higher patient census, or other client-care needs. Temporary reassignments may be necessary to maintain safe, high-quality care. The Case Manager creates, implements, and executes patients’ overall plans of care to promote wellness and prevent rehospitalization. The RN oversees assistance and teaching for patients and their families or caregivers, including wound care, blood draws, tube feedings, tracheostomy care, Foley catheter care, congestive heart failure management, and medication education. The role includes patient and family assessment, care planning, one-on-one work toward improvement or stability, case management throughout the care episode, timely and compliant clinical documentation, and collaboration with physicians and agency staff. The position offers administrative support for case managers, flexible scheduling, targeted territories, autonomy, advancement opportunities, and a quarterly incentive bonus program. Most RN Case Managers are initially hired as RNIs. Promotion to RNII may occur when an RNI can supervise two LVNs, manage a larger caseload, and take on greater decision-making and autonomy. RNII compensation is stated as a minimum salary of $135,000 plus a $20,000 quarterly-paid bonus.
