About The Position

21st Century Home Health Services is hiring a full-time Registered Nurse, Level 1, to provide home health case management in Alameda County, with an anticipated coverage area of Oakland and surrounding cities. The role requires field work and visits to patients in their homes. The schedule is Tuesday–Saturday or Sunday–Thursday, and the nurse must be willing to work either Saturday or Sunday. The position has a primary territory, but occasional coverage outside that area may be required to support vacancies, leaves of absence, higher patient census, or other client care needs. The nurse creates, implements, and executes plans of care to promote wellness and prevent rehospitalization. They assess patients and families, provide or oversee assistance and teaching related to clinical needs, and work with patients and caregivers toward improvement or stability. The role case-manages each care episode in collaboration with an interdisciplinary clinical team, physicians, and agency staff, and completes clinical notes, orders, and charting within compliant time frames. The organization describes its model as combining home health care with structure, tools, resources, and clinician support. It offers administrative assistance to case managers, flexible scheduling, targeted territories, autonomy, advancement opportunities, and professional development. Most RN case managers are hired as RNIs.
