About The Position
We believe that the best clinical care happens where patients feel safest: at home. As a field Registered Nurse (RN), you won’t just manage symptoms—you will be the clinical lead for your patients, promoting comfort, dignity, and independence when it matters most.
Requirements
- Current Texas RN license and BLS/CPR certification.
- 1+ years of clinical nursing experience (Hospice preferred).
- Reliable vehicle, valid TX driver’s license, and auto insurance.
Nice To Haves
- Bilingual (English/Spanish) is a major plus.
Responsibilities
- Conduct comprehensive head-to-toe patient assessments and build customized care plans tailored to individual goals.
- Perform wound care, IV therapy, symptom management, and medication reconciliation in the field.
- Serve as the primary clinical liaison, coordinating directly with physicians, families, and our interdisciplinary team.
- Provide vital patient and caregiver education on disease progression and emotional guidance through complex care journeys.
- Utilize EMR systems to ensure timely, accurate charting that meets regulatory compliance.
