About The Position

This is a 13-week travel assignment in a hospital-based home health care setting. The role involves providing skilled nursing care to home health patients, including assessment, ongoing monitoring, and developing individualized care plans. Responsibilities include performing OASIS visits, administering medications, educating patients and caregivers, and documenting all care accurately. The position requires participation in a weekend rotation.

Requirements

Active Registered Nurse license in the State of Maine
BLS certification
Minimum of 1 year of Home Health Care experience
Ability to perform OASIS visits
Weekend rotation required (approximately 2–3 weekend days per month)
Able to work in a home health setting with appropriate safety and infection control practices

Responsibilities

Provide skilled nursing care to home health patients, including assessment and ongoing monitoring
Perform OASIS visits and data collection as part of patient care planning and regulatory compliance
Develop, implement, and adjust individualized care plans in collaboration with the interprofessional team
Administer medications and deliver appropriate clinical interventions within scope of practice
Educate patients and caregivers on disease management, safety, and treatment plans
Document all care and observations accurately in the patient record; ensure compliance with policies and regulations
Coordinate with other providers and services to ensure smooth transitions and timely follow-up
Participate in a weekend rotation as scheduled
