About The Position

This is a travel assignment for a Home Health Registered Nurse specializing in Hospice care in Rutland, Vermont. The role involves providing quality home-based hospice care, developing individualized care plans, serving as a Case Manager, completing accurate OASIS documentation, and implementing the medical plan of care. The nurse will also collaborate with LPNs and other disciplines to ensure comprehensive patient care.

Requirements

OASIS experience required
Hospice coverage while on call required
Willingness to participate in on-call, weekend, and holiday rotations
Ability to perform high-tech nursing skills (IV use, wound care, etc.) and maintain infection control
Proficient in accurate and timely nursing documentation per agency/state/federal guidelines
Experience in home health or hospice settings preferred and knowledge of Vermont State Board of Nursing policies

Responsibilities

Use the nursing process to assess patient and caregiver needs and deliver quality home-based hospice care in line with evidence-based practice and applicable regulations.
Develop and communicate individualized care plans with patient/caregiver involvement and coordinate with the interdisciplinary team.
Serve as Case Manager for assigned patients; actively manage caseload, visit times, and compliance with agency productivity standards.
Complete accurate OASIS documentation per agency policies and state/federal regulations; stay current on changes to OASIS requirements.
Implement the medical plan of care and nursing interventions; maintain high-tech skills (IVs, wound care, wound VAC) while upholding infection control standards.
Collaborate with LPNs and other disciplines to ensure plan of care implementation, document visits, and communicate recommendations in a timely manner.
