About The Position

This is a travel Registered Nurse position specializing in Geriatric care, located in Saint Helena, California. The role is based in a Short Term Acute Care Hospital, specifically on two adult MHU units (Medical Behavioral Health and Adult Psychiatry). The contract duration is 13 weeks, with a start date of 09/28/2026. The position guarantees 40 hours per week, with a day shift schedule consisting of five 8-hour shifts from 08:00 to 16:30. The estimated weekly pay ranges from $2,849 to $2,959.

Requirements

Current Registered Nurse license in California (state of assignment)
Basic Life Support (BLS) certification
Epic experience required
Minimum two years of recent nursing experience in the field (care management or related case management experience preferred)
Ability to work in an acute care setting and on assigned units; no on-call

Case management certification preferred

Responsibilities

Lead the coordination of patient care with the interdisciplinary team, developing, implementing, revising, and reporting the case management plan.
Serve as a liaison among patient, family, nurse, physicians, and the care team to optimize outcomes and ensure a smooth discharge plan.
Monitor the appropriateness and timeliness of diagnostic testing, treatments, and resource utilization; help determine estimated length of stay and discharge needs.
Focus on complex patients, frequent ED utilizers, chronic pain patients, substance use, and psychosocial challenges; perform focus assessments to guide care.
Coordinate transfers to tertiary centers when needed and ensure the transfer of information required for continuity of ongoing treatment.
Provide oversight and collect data required for regulatory and accreditation compliance; support ongoing case management activities.
No on-call requirement.
