About The Position
Interim HealthCare is hiring a Home Health Registered Nurse to provide one-on-one, home-based care to patients in Wichita, Kansas. The role serves patients with illnesses, injuries, and chronic diseases, often following discharge from a hospital, rehabilitation center, or nursing facility. Nurses work with physicians and oversee LPNs, CNAs, and aides involved in patient care, while directing their own day through flexible assignments and autonomous work.
Requirements
- Graduate of an accredited nursing program.
- Active RN license in Kansas.
- CPR certification.
- Clinical proficiency in a nursing specialty.
- Strong communication skills and compassion.
Responsibilities
- Provide home-based care to patients with illnesses, injuries, and chronic diseases, often after discharge from a hospital, rehabilitation center, or nursing facility.
- Work with a team of physicians and oversee LPNs, CNAs, and aides providing patient care.
- Examine patients, conduct assessments, document progress, and report changes to physicians.
- Educate patients about their plan of care, prescribed medication, therapy, diet, and exercise.
- Coach family members on the patient’s plan of care, medication, and home safety.
- Ensure patient goals are met and coordinate discharge from services.
