Home Health Registered Nurse Case Manager
About The Position
As a CenterWell Home Health nurse, you will report to the Clinical Manager and provide personalized, one-on-one skilled nursing care to patients in their homes. You will work with physicians and other clinicians to manage care plans that support recovery and help patients regain independence. The role includes patient assessments, medication administration, wound care, monitoring patient conditions, education for patients and families, maintaining care records, coordinating with healthcare professionals, reporting progress to physicians and the Clinical Manager, and overseeing Home Health Aides.
Requirements
- Med-surg, ICU, ER, or acute experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver's license, auto insurance, and reliable transportation.
Nice To Haves
- One year of nursing experience.
- Home Health experience.
Responsibilities
- Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients' conditions and report changes.
- Educate patients and their families on disease management, medication, and treatment options.
- Maintain accurate records of patient care and coordinate with other healthcare professionals.
- Report patient care and condition progress to the patient's physician and Clinical Manager.
- Oversee the work of Home Health Aides and monitor appropriate completion of documentation.
