About The Position
As a CenterWell Home Health nurse, reporting to the Clinical Manager, you provide personalized, one-on-one care to help patients regain independence in their homes. Working with physicians and other clinicians, you manage care plans that support recovery. The position involves skilled nursing care in patients’ homes, including medication administration, wound care, and patient assessments. You develop individualized care plans, monitor patient conditions, educate patients and families, maintain records, coordinate care, report progress, and oversee Home Health Aides’ work and documentation.
Requirements
- Med-surg, ICU, ER, or acute experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver’s license, auto insurance, and reliable transportation.

- One year of nursing experience.

Responsibilities
- Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients’ conditions and report changes.
- Educate patients and their families on disease management, medication, and treatment options.
- Maintain accurate patient-care records and coordinate with other healthcare professionals.
- Report patient care and condition progress to the patient’s physician and Clinical Manager.
- Oversee Home Health Aides and monitor the appropriate completion of their documentation.
