About The Position

As a CenterWell Home Health nurse, reporting to the Clinical Manager, you provide personalized, one-on-one care to help patients regain independence at home. Working with physicians and clinicians, you manage care plans that support recovery. The role is patient-facing and involves providing skilled nursing care in patients’ homes, including medication administration, wound care, and assessments. It also includes care-plan development, patient and family education, care coordination, accurate documentation, reporting patient progress, and oversight of Home Health Aides.

Requirements

Med-surg, ICU, ER, or acute care experience.
Current and unrestricted Registered Nurse licensure.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Valid driver’s license, auto insurance, and reliable transportation.

Nice To Haves

One year of nursing experience.
Home Health experience.

Responsibilities

Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments.
Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
Monitor patients’ conditions and report changes.
Educate patients and families about disease management, medication, and treatment options.
Maintain accurate records of patient care and coordinate with other healthcare professionals.
Report patient care and condition progress to the patient’s physician and Clinical Manager.
Oversee Home Health Aides and monitor their appropriate completion of documentation.
