About The Position

As a CenterWell Home Health nurse reporting to the Clinical Manager, you will provide personalized, one-on-one care to help patients recover and regain independence in their homes. You will work with physicians and other clinicians to manage care plans that support patients’ recovery. The role involves providing skilled nursing care in patients’ homes, coordinating treatment, educating patients and families, documenting care, and overseeing Home Health Aides.

Requirements

• Med-surg, ICU, ER, or acute-care experience.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver’s license, auto insurance, and reliable transportation.

Nice To Haves

• One year of nursing experience.
• Home Health experience.

Responsibilities

• Provide skilled nursing care in patients’ homes, including administering medications, performing wound care, and conducting patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients’ conditions and report changes.
• Educate patients and their families on disease management, medication, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care and condition progress to the patient’s physician and the Clinical Manager.
• Oversee Home Health Aides and monitor appropriate completion of their documentation.
