Home Health Registered Nurse Case Manager
About The Position
As a CenterWell Home Health nurse reporting to the Clinical Manager, you provide personalized, one-on-one care to help patients regain independence at home. You work with physicians and other clinicians to manage care plans that support recovery. The role provides skilled nursing care in patients' homes, including medication administration, wound care, and patient assessments. You develop and implement individualized care plans with healthcare professionals, monitor patients and report changes, educate patients and families, maintain accurate care records, coordinate care, and report progress to physicians and the Clinical Manager. You also oversee Home Health Aides and monitor completion of their documentation.
Requirements
- Med-surg, ICU, ER, or acute experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver's license, auto insurance, and reliable transportation.

Responsibilities
