About The Position

Interim HealthCare is hiring a Home Health Registered Nurse in Wichita, Kansas. The role provides personalized, one-on-one nursing care in patients’ homes, often following discharge from a hospital, rehabilitation center, or nursing facility. Nurses have flexible assignments and autonomy in organizing their day while working with a care team to support patient outcomes. The nurse assesses patients, documents progress, reports changes to physicians, and educates patients and family members about care plans, medications, therapy, diet, exercise, and home safety. The role also includes coordinating care and discharge from services. The position works collaboratively with physicians and oversees LPNs, CNAs, and aides involved in patient care.

Requirements

Graduate of an accredited nursing program.
Active RN license in Kansas.
CPR certification.
Clinical proficiency in a nursing specialty.
Strong communication skills and compassion.

Responsibilities

Provide home-based care to patients with illnesses, injuries, and chronic diseases, often after discharge from a hospital, rehabilitation center, or nursing facility.
Work with physicians and oversee LPNs, CNAs, and aides providing patient care.
Examine patients, conduct assessments, document progress, and report changes to physicians.
Educate patients about their care plans, medications, therapy, diet, and exercise.
Coach family members on the patient’s care plan, medication, and home safety.
Ensure patient goals are met and coordinate discharge from services.
