Registered Nurse - Behavioral Health Case Manager
About The Position
Spectrum Healthcare Resources is hiring a full-time Registered Nurse - Behavioral Health Case Manager at the Fairfax Ambulatory Care Clinic in Fairfax, Virginia. This position is based in an outpatient healthcare setting and has a 40-hour workweek, Monday through Friday, from 0730 to 1600. The role provides an opportunity to deliver healthcare resources to the nation's military community. The nurse triages patients with behavioral health concerns, helps coordinate appointments and emergent care, and collaborates with the behavioral health provider team and other healthcare staff to coordinate patient care. The position also supports high-risk patient monitoring and helps patients access community resources and ongoing case management assistance. The position has no on-call, weekend, or holiday requirements and includes 11 paid federal holidays off. The stated pay range is $38.95–$43.88 per hour. Requirements
- Must be a U.S. citizen.
- Bachelor of Science in Nursing (BSN) required.
- At least one year of Behavioral Health or Case Manager RN experience.
Nice To Haves
- Behavioral Health or Case Manager RN experience preferably gained within the last five years.
Responsibilities
- Triage patients by gathering relevant information to assess the nature and urgency of behavioral health concerns.
- Assist patients in scheduling appointments within the direct and purchased care behavioral health system.
- Facilitate emergent care coordination and transfers for inpatient psychiatric hospitalization.
- Collaborate with other healthcare team members to develop and coordinate comprehensive patient care plans.
- Work with the behavioral health provider team to monitor, track, and manage patients identified as high risk.
- Serve as a resource person and facilitator to help patients access available community resources.
- Collaborate with Health Center Case Management staff to support case management tracking and assistance for behavioral health patients.
