About The Position

ViaQuest Psychiatric & Behavioral Solutions provides individualized care to children, adolescents, adults, older adults, people with developmental disabilities, and people experiencing extreme trauma. Its team includes mental health counselors, social workers, certified nurse practitioners, case managers, and psychiatrists. The Behavioral Health Case Manager provides therapeutic behavioral services and individualized planning and support to clients, in consultation with a licensed clinician. The role serves individual clients or groups and requires travel throughout an assigned service area.

Requirements

• Must be at least 18 years of age.
• Have a bachelor's or master's degree in a related field, a high school diploma, or three or more years of relevant field experience.
• Related experience or training with mental or behavioral health conditions.
• Valid driver's license, car insurance, and reliable transportation.
• Willingness to travel throughout the assigned service area.

Responsibilities

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with individual needs.
• Plan individualized supports and coordinate healthcare, behavioral health, and non-healthcare services, including development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills, and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with outside contacts.
