Registered Nurse Assessor - Field in Mercer and Middlesex County, NJ
About The Position
The RN Field Assessor conducts face-to-face assessments with members to evaluate medical needs, support overall wellness, and help members obtain services that enable them to thrive. Assessments may address requests for adult or pediatric medical daycare, personal care assistants, nursing facility custodial services, the personal preference program, and enrollment in the MLTSS program. The role completes a minimum of 15–20 assessments per week, depending on need, and may complete more. Assessments draw on information from multiple sources to evaluate conditions, including comorbidities and multiple diagnoses that affect functionality, as well as the member’s work capacity and related restrictions or limitations. The position requires substantial field travel within Mercer and Middlesex County, New Jersey, and the ability to work Monday through Friday, 8:00 a.m. to 5:00 p.m. EST. Mileage is reimbursed under the company’s expense reimbursement policy. Successful completion of NJ Choice certification is a contingency of employment.
Requirements
- Active, unrestricted Registered Nurse license in New Jersey.
- Reliable transportation and willingness and ability to travel up to 85% of the time.
- At least 2 years of clinical practice experience in a hospital, alternative care, home health, or ambulatory care setting.
- Successful completion of NJ Choice certification as a contingency of employment, including an exam score of 80% or higher, successful completion of State training modules, and the field mentoring component.
- Ability to work Monday through Friday, 8:00 a.m. to 5:00 p.m. EST.
- Experience using a personal computer, keyboard navigation, multiple systems and applications, and MS Office Suite applications such as Teams, Outlook, Word, and Excel.
- Associate degree in Nursing.
Nice To Haves
- Bachelor of Science in Nursing.
- Assessment, case management, managed care, and/or discharge planning experience.
- Crisis intervention experience.
- Clinical interviewing experience.
