About The Position
Foundation Medical Partners, affiliated with Southern New Hampshire Medical Center, is a multi-specialty group serving the greater Nashua community. Its Immediate Care service provides walk-in care for patients of all ages with non-life-threatening illnesses and injuries, and coordinates with patients’ primary care physicians. The Registered Nurse assists providers and delivers nursing care in person, by telephone, and through telehealth. Duties include rooming patients, taking vital signs, triaging, calling in prescriptions, and other associated responsibilities. The nurse assesses patient needs, plans and evaluates care, collaborates with an interdisciplinary team, administers medications and treatments, performs point-of-care testing, handles specimens, and educates patients on self-management and wellness. This is a part-time position working every other weekend from 9:00 a.m. to 6:00 p.m.
Requirements
- Current New Hampshire Registered Nurse (RN) license.
- Current BLS/CPR training.
- Prior work experience in a healthcare setting.

Responsibilities
- Assist providers as necessary.
- Room patients, take vital signs, triage patients, call in prescriptions, and perform associated responsibilities.
- Carry out the nursing process in person, by telephone, including telephone triage, and through telehealth modalities.
- Assess and identify patient needs, plan to achieve appropriate outcomes, intervene, and evaluate progress toward goals of care.
- Collaborate with physicians, physician assistants, and other health professionals to design and implement a plan of care.
- Administer prescribed medications, treatments, and vaccines within the scope of practice.
- Perform point-of-care testing and collect and handle specimens within the scope of practice.
- Educate patients on self-management activities, medications, nutrition, health, and wellness based on the collaboratively designed plan of care.
