Requirements

  • High School Diploma
  • 1 Year Administrative Healthcare Experience

Nice To Haves

  • Bachelor’s Degree
  • 1-3 years Experiences, preferably in HomeCare

Responsibilities

  • Performs all daily patient referral and intake procedures including insurance verification and financial/clinical clearance and ensures patients are admitted in accordance with the intake policy.
  • Provides recommendations for process and policy improvements.
  • Ensures compliance with all state, federal, and Joint Commission referral/intake regulatory requirements.
  • Stays thoroughly informed about the details of contracts the organization has with insurance companies, healthcare networks, or other partners for Home Health and Hospice services.
  • Ensures patient admissions comply with rules, requirements and terms in the contracts.
  • Establishes and maintains positive working relationships with current and potential referral sources.
  • Provides a smooth transition of patients to home care by overseeing patient education, care planning, and coordination with multiple providers, while also negotiating service pricing with insurance Case Managers and other payers within established financial guidelines.
  • Strengthens and monitors community and customer perceptions of Hartford Healthcare at Home as a high-quality service provider, while collecting and reporting referral data and key customer trends.
  • Performs the entire patient intake process, including communication with patients and families, data entry, insurance verification and MD verification.
  • Builds and maintains relationships with referral sources.
  • Addresses customer needs.
  • Leads a daily intake meeting.
