About The Position

Sycamores provides a spectrum of equitable mental health services through various programs to multicultural populations of all acuities. Although it serves a variety of consumers, the agency focuses primarily on providing care to low-income, high at-risk youth. The Referral Specialist is responsible for assisting with managing and processing referrals. The Referral Specialist will utilize their expertise to determine appropriate resources or services for the consumer and family while collaborating with community partners. This position will be responsible for maintaining a consistent referral flow for the department through excellent customer service within our community. The Referral Specialist will refer parties and youth/young adult and family according to the guiding principles of the agency’s philosophy of strength based, family focused needs driven and culturally competent.

Requirements

  • Bachelor’s degree required.
  • Maintains all required licenses and certifications.
  • Valid California Driver License and a driving record acceptable to the Agency’s insurance carrier.
  • All candidates who are presented with a conditional offer of employment must satisfy a pre-employment background check, drug, and health screen.

Nice To Haves

  • At least 2 years of relevant experience are preferred.
  • Bi-lingual Spanish preferred.

Responsibilities

  • Receive and screen consumer referrals for the agency, gathering pertinent consumer information to determine eligibility for agency programs and services.
  • Processing and assigning all consumer referrals to agency designated teams.
  • Track and manage all consumer referrals.
  • Keep updated on capacity and wait list throughout the organization so that appropriate referrals can be expedited.
  • Enter pertinent information in Electronic Health Record system, SRTS, WTS and SRL.
  • Communicate and collaborate with Clinical Supervisors and Program Director to ensure effective referral process and protocols.
  • Tracking and updating required client list as needed for DMH and programs.
  • Keep abreast of all agency programs and services to make the appropriate referrals for the agency.
  • Maintain direct and positive communication with potential consumers and referring parties to build a positive reputation and resource.
  • Mail, Fax, Scan intake documents for sites and referring agencies.
  • Attend all agency meetings and trainings as needed.
  • Participate in regular supervision meetings as requested by supervisor.
  • Perform other duties as assigned.
