About The Position

The Project Manager - Rural Access & Care Coordination provides implementation leadership to move the approved RHT Initiative 6.2 and CCBHC Bridge Funding plan from funding request to coordinated operational practice. Reporting directly to the Executive Vice President of Outpatient Services, the role translates approved grant scope, annual funding requirements, and deliverables into an integrated implementation plan with milestones, accountable owners, dependencies, RACI assignments, monitoring, and documented results. The Project Manager coordinates internal teams, vendors, consultants, and community partners to implement rural access, care coordination, workflow, workforce, data, CCBHC readiness, and sustainability activities within approved timelines and budgets. The position manages the project life cycle from initiation and planning through implementation, operational transition, annual renewal activities, and grant closeout. It coordinates the design, documentation, training, implementation, and adoption of grant-funded workflows and data infrastructure; supports CCBHC readiness, quality, compliance, and grant-review preparation; and maintains implementation controls, reporting, and evidence. The role also ensures activities remain within the authorized grant purpose and that major grant-funded work has operational owners and sustainability plans before transition to routine operations. The position is based at the Yakima Main Center in Yakima, Washington, and is primarily office-based, with interactions across departments, service locations, vendors, consultants, and community partners. Travel between sites and occasional schedule flexibility may be required. This is a grant-funded position supported by RHT Initiative 6.2 and CCBHC Bridge Funding. The anticipated grant period begins October 1, 2026 and extends for five years, with funding requested and approved annually. Continued employment depends on annual funding availability, approved grant scope, satisfactory grant performance, and organizational need.
