Program Support Assistant
About The Position
The case management associate participates on the Case Management Team (CMT) and, with guidance from the nurse case manager and social worker, assists in facilitating appropriate patient placement. The role uses hospital and community resources to support continuity of patient care, serves as a Utilization Management liaison, and collaborates with medical staff, CMT members, the Business Office, Medical Records, and other hospital staff. Other duties may be assigned. Requirements
- At least 1 year of job-related experience in a healthcare setting.
Nice To Haves
- Associate's degree.
- Bachelor's degree.
- Nursing Assistant Certification (CNA), Health Unit Coordinator (HUC), or medical terminology upon hire.
- Prior experience in discharge planning, billing, utilization review, financial counseling, or registration.
- Proficiency in medical terminology.
Responsibilities
