About The Position
Valley View is looking for RN Case Managers with experience in hospital and Epic preferred! We are looking for a PRN Case Manager RN to join our Case Management team! We will hire a Registered Nurse or MSW for hospital discharge planners onsite at Valley View Hospital. Case Management and Epic experience a plus! Previous Case Management experience strongly preferred!
Requirements
- Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program
- State licensure as a Registered Nurse (RN).
- 1 year of nursing (RN) experience
- Computer experience (word processing, spreadsheet) and familiarity with Meditech

Nice To Haves
- BSN RN preferred.
- Case management experience preferred.
- Experience in hospital and Epic preferred!
Responsibilities
- Assists Valley View inpatients and outpatients and their families in the overall Management of their plan of care across the continuum.
