RN Case Manager (PRN)

About The Position
The RN Case Manager collaborates with patients, families, and the healthcare team to develop discharge plans and coordinate necessary post-acute care services. The role also assists with gathering data to ensure patient care meets approved admission criteria, continued-stay requirements, and appropriate levels of care. This is a PRN position with an hourly rate based on experience: $69.00 for 0–5 years and $71.00 for 6 or more years.
Requirements
- Associate Degree in Nursing (ADN).
- Current California Registered Nurse (RN) license.
- Current Basic Life Support (BLS) certification.
- One year of case management experience (RN CM II).
Nice To Haves
- Bachelor of Science in Nursing (BSN).
- Bilingual in Spanish.
- One year of experience with InterQual.
- One year of experience with Allscripts, Meditech, or Epic.
