PRN RN Case Manager
About The Position
Lifted Hospice seeks a PRN Registered Nurse Case Manager to provide compassionate, clinically excellent hospice care to patients and families facing end-of-life. The role delivers personalized nursing care in patients’ homes or care settings, manages symptoms, and supports quality of life. The RN Case Manager collaborates with the interdisciplinary team to develop and follow care plans that reflect patient goals and meet clinical, ethical, and emotional care standards. The position also educates and supports families and caregivers so they understand what to expect and feel prepared to provide comfort. Most of the work takes place wherever patients call home, including private residences, memory care facilities, and other long-term care facilities. The nurse meets with the local care team, including social workers, chaplains, physicians, fellow nurses, and the medical director, bi-weekly to align patient care plans. Requirements
- Graduate of an accredited Diploma, Associate, or Bachelor’s nursing program.
- Active state nursing license and clinical skills meeting current standards of nursing practice.
- Experience as a registered nurse in a clinical care setting.
- Excellent interpersonal skills.
- Ability to read, write, and comprehend English.
- Current driver’s license, reliable transportation, and valid auto liability insurance.

Responsibilities
- Complete thorough assessments of patients’ physical, emotional, and clinical needs to guide care.
- Deliver skilled hospice nursing care to manage symptoms and pain and improve quality of life.
- Collaborate with the care team, patients, and their loved ones to create and follow a unified care plan.
- Document care and updates accurately in accordance with standards and requirements.
- Educate and encourage caregivers, provide emotional support, and help families feel prepared and confident.
- Meet bi-weekly with the local care team to align patient care plans.
