About The Position

Valley View is looking for RN Case Managers with experience in hospital and Epic preferred! Days/Rotating weekends! We're looking for a PRN Case Manager RN to join our Case Management team! We will hire a Registered Nurse or MSW for hospital discharge planners onsite at Valley View Hospital. Case Management and Epic experience a plus! Previous Case Management experience strongly preferred! GENERAL OBJECTIVES: Assists Valley View inpatients and outpatients and their families in the overall Management of their plan of care across the continuum.

Requirements

• Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program
• State licensure as a Registered Nurse (RN).
• 1 year of nursing (RN) experience
• Computer experience (word processing, spreadsheet) and familiarity with Meditech
• Eligible to work in the United States.

Nice To Haves

• BSN RN preferred.
• Case management experience preferred.
• Epic preferred.
• Previous Case Management experience strongly preferred!
• MSW
