About The Position

Phoenix Home Care & Hospice is seeking a part-time Private Duty Assistant Nursing Supervisor (LPN) to support its pediatric and young adult nursing team in Springfield and surrounding communities. The role combines leadership and clinical support with hands-on involvement in patient care. The supervisor helps nurses, clients, and families succeed in a fast-paced home care environment through training, field visits, and in-office support.

Requirements

• Current LPN license held for at least 2 years.
• Organized, energetic, and self-motivated.
• Strong communication and leadership skills.
• Comfortable working independently in the field and as part of a team.
• Passion for supporting pediatric and young adult patients and their families.

Nice To Haves

• Trach, vent, G-tube, and J-tube experience.

Responsibilities

• Provide clinical support and leadership to the Private Duty Nursing team.
• Orient and train nurses, clients, and families.
• Conduct field nursing visits and provide in-office support.
• Help ensure nurses have the tools, training, and support they need to succeed.
• Work closely with Care Coordinators and other clinical team members.
• Provide guidance and support in a fast-paced home care environment.
