About The Position
Elk Valley, part of LHC Group and the Optum family of businesses, provides home health, hospice, and community-based services. The Licensed Practical Nurse provides physician-prescribed direct nursing care to acutely or chronically ill patients in their homes. The role records clinical and progress notes and coordinates other patient care activities and services to support high-quality care in the assigned geographic service area. The LPN works under the direct supervision of a Registered Nurse and/or designated supervisor.
Requirements
- Current and unrestricted Licensed Practical Nurse licensure in the state of practice.
- Current driver’s license and vehicle insurance, access to a dependable vehicle, or access to public transportation.
Nice To Haves
- Current CPR certification or ability to complete certification within 90 days of hire.
- Ability to work flexible hours as required to meet identified client needs.
- Ability to manage multiple tasks simultaneously.
- Ability to work independently.
- Good communication, writing, and organizational skills.
Responsibilities
- Provide high-quality clinical services within the scope of practice defined by state nursing laws, in accordance with the plan of care and in coordination with other members of the health care team.
- Follow applicable infection control procedures.
- Thoroughly and completely document patient visits in the electronic medical record according to regulatory and payer requirements, using the agency’s electronic visit verification program when applicable.
- Participate in the agency’s care coordination process, including timely reporting of patient needs and changes in condition and attending patient case conferences.
- Communicate effectively with patients, families, physicians, other clinicians, agency management staff, case coordinators, and community case managers when applicable.
- Function in home situations regardless of a client’s age, race, creed, color, sex, disability, or financial condition.
