About The Position

WelbeHealth PACE provides seniors with comprehensive medical services designed to help them continue living in their homes and communities. The organization uses a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care. At the direction of the Medical Director, the Primary Care Provider collaborates with WelbeHealth Physicians and the IDT to complete assessments, plan primary care, and educate participants and their families. Providers have a smaller patient caseload, allowing them to build long-term relationships and practice value-based care. The role provides continuous medical coverage and comprehensive in-person assessments, including initial, semi-annual, and unscheduled assessments. The provider collaborates on participant care plans and care coordination, arranges discharge planning, supports care in skilled nursing facilities and residences, and reviews outcomes and utilization data to identify best practices and policy improvements. The position requires participation in a rotating night and weekend call schedule. Compensation consists of base salary and bonus, with a stated range of $150,009.60–$190,216 USD.
