About The Position

WelbeHealth PACE (Program of All-Inclusive Care for the Elderly) provides seniors with medical services designed to help them continue living in their homes and communities. The organization uses a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care. At the direction of the Medical Director, the Primary Care Provider works with WelbeHealth Physicians and the IDT to complete assessments, plan primary care, and educate participants and their families. Providers serve a smaller patient caseload, enabling long-term relationships and value-based care, and collaborate with providers, therapists, and other disciplines. The role provides continuous medical coverage and comprehensive in-person assessments, including initial, semi-annual, and unscheduled assessments. The provider helps develop care plans, coordinate care, arrange discharge planning, maintain care in skilled nursing facilities and residences, and review outcomes and utilization data to inform best practices and policy improvements. The role includes rotating night and weekend call. Compensation consists of base salary plus bonus, with a stated range of $150,009.60–$190,216 USD. Actual compensation depends on experience and relevant qualifications. WelbeHealth requires employees to provide proof of vaccination or a vaccine declination form before employment and to follow infection-control and PPE requirements.
