About The Position

WelbeHealth PACE (Program of All-Inclusive Care for the Elderly) provides seniors with the opportunity to continue living in their homes and communities through comprehensive medical services. Using a collaborative interdisciplinary team (IDT) approach, the Primary Care Provider works at the direction of the Medical Director with WelbeHealth Physicians and the IDT to assess participants, plan primary care, and educate participants and their families.
