About The Position

Millennium Physician Group, Florida's largest independent physician group, is seeking a Primary Care Physician for Lake Placid, FL. This role offers physician independence, allowing clinical decisions to remain with the physician without hospital or insurance company employment. The focus is on community medicine, particularly chronic disease management and prevention within a Medicare Advantage population. The position features a Monday through Friday schedule, providing predictable hours and work-life balance. Millennium handles administrative burdens through its platform, Millennium CARES Connect, including payer contracting, quality and coding, HCC risk adjustment, credentialing, billing, and population health management. Clinical support is provided by a 24/7 nurse triage line, case managers, hospitalists, transitional care coordinators, social workers, discharge planners, palliative care specialists, clinical pharmacists, and home health. The technology stack includes AthenaOne EMR with AI integration to reduce documentation time, and integrated Virtual Care Clinic and Telehealth services.

Requirements

MD or DO degree with completed residency in Family Medicine or Internal Medicine
Board Certified or Board Eligible
Active, unrestricted state medical license (or eligible to obtain) and valid DEA number
EMR proficiency
Commitment to value-based, patient-centered, outcomes-driven care
