About The Position

The Primary Care Physician delivers high-quality, value-based care to an established panel of predominantly geriatric patients in a 100% outpatient setting. The role emphasizes preventive medicine, chronic disease management, and meaningful long-term patient relationships. The physician works within a collaborative, multidisciplinary care model focused on improving patient outcomes, enhancing access to care, and reducing unnecessary hospitalizations through personalized care plans, proactive health management, and patient education. The position is full-time, Monday through Friday, 8:00 AM to 5:00 PM, with no weekends or hospital rounds. The physician-led, team-oriented environment prioritizes quality over volume and provides clinical and administrative support.

Requirements

MD or DO from an accredited medical school.
Training in Internal Medicine, Family Medicine, or Geriatric Medicine.
Active Alabama (AL) license.
Current DEA registration.
At least 3 years of experience in outpatient primary care.
Strong clinical judgment focused on preventive and longitudinal care.
Excellent interpersonal and communication skills.
Commitment to a team-based, patient-first care model.

Responsibilities

Provide comprehensive primary care, including evaluation, diagnosis, and treatment of acute and chronic conditions.
Deliver preventive care, wellness visits, and health screenings, with a focus on early intervention.
Develop and manage individualized care plans for a predominantly geriatric population.
Collaborate with nurse practitioners, care coordinators, and support staff to provide seamless, patient-centered care.
Use electronic health records (EHR) to document clinical encounters and track patient outcomes.
Participate in quality improvement initiatives and contribute to continuous care enhancement.
Educate patients and families about disease prevention, treatment plans, and healthy lifestyle choices.
