About The Position

We are seeking a compassionate and dedicated Primary Care Physician to provide high-quality, value-based care to an established panel of predominantly geriatric patients. This full-time, 100% outpatient position emphasizes preventive medicine, chronic disease management, and meaningful, long-term patient relationships. The physician works within a collaborative, multidisciplinary care model focused on improving patient outcomes, increasing access to care, and reducing unnecessary hospitalizations. Care is personalized and proactive, with an emphasis on individual care plans and patient education. The schedule is Monday through Friday, 8:00 AM to 5:00 PM, with no weekends or hospital rounds.

Requirements

MD or DO from an accredited medical school.
Training in Internal Medicine, Family Medicine, or Geriatric Medicine.
Active Florida license.
Current DEA registration.
At least 3 years of experience in outpatient primary care.
Strong clinical judgment focused on preventive and longitudinal care.
Excellent interpersonal and communication skills.
Commitment to a team-based, patient-first care model.

Responsibilities

Provide comprehensive primary care, including evaluation, diagnosis, and treatment of acute and chronic conditions.
Deliver preventive care, wellness visits, and health screenings, with a focus on early intervention.
Develop and manage individualized care plans for a predominantly geriatric population.
Collaborate with nurse practitioners, care coordinators, and support staff to provide seamless, patient-centered care.
Use electronic health records (EHR) to document clinical encounters and track patient outcomes.
Participate in quality improvement initiatives and contribute to continuous care enhancement.
Educate patients and families about disease prevention, treatment plans, and healthy lifestyle choices.
