Join an innovative, mission-driven organization redefining primary care for seniors through compassionate, relationship-centered, value-based medicine. The Primary Care Physician will join the growing Montgomery team following a physician transition and provide care to Medicare-eligible adults and underserved senior communities. The care model features smaller patient panels, interdisciplinary collaboration, and proactive, wellness-focused care, with an experienced clinical and operational team supporting physicians in improving outcomes, reducing barriers to care, and strengthening communities. The position offers a modern, technology-enabled practice environment with on-site diagnostics, including labs, EKG, and echocardiogram. Physicians work with an interdisciplinary team that includes nurses, MAs, case managers, social workers, and community outreach specialists, as well as dedicated support for care coordination, patient navigation, and population health management. Continuous professional development and training in value-based care are provided. Compensation includes a base salary ranging from $250,000 for new graduates or residents up to $295,000 for experienced physicians, an annual incentive bonus with recent physician payouts ranging from $45,000 to $85,000, and a $15,000 sign-on bonus. Benefits include medical, dental, and vision insurance; generous PTO and CME allowance; short- and long-term disability coverage; life insurance equal to one year of base salary; and malpractice coverage including tail coverage.
