Primary Care Physician - Sign on Bonus

About The Position

We are seeking a compassionate and dedicated Primary Care Physician to deliver high-quality, value-based care to an established panel of predominantly geriatric patients. The role emphasizes preventive medicine, chronic disease management, and meaningful, long-term patient relationships in a supportive outpatient environment. The physician will work within a collaborative, multidisciplinary care model designed to improve patient outcomes, enhance access to care, and reduce unnecessary hospitalizations. The approach prioritizes personalized care plans, proactive health management, and patient education. This is a full-time, 100% outpatient position focused on geriatrics, with a Monday–Friday schedule from 8:00 AM to 5:00 PM and no weekends or hospital rounds.

Requirements

MD or DO from an accredited medical school.
Training in Internal Medicine, Family Medicine, or Geriatric Medicine.
Active Florida license.
Current DEA registration.
At least 3 years of experience in outpatient primary care.
Strong clinical judgment focused on preventive and longitudinal care.
Excellent interpersonal and communication skills.
Commitment to a team-based, patient-first care model.

Responsibilities

Provide comprehensive primary care services, including evaluation, diagnosis, and treatment of acute and chronic conditions.
Deliver preventive care, wellness visits, and health screenings, focusing on early intervention.
Develop and manage individualized care plans for a predominantly geriatric population.
Collaborate with nurse practitioners, care coordinators, and support staff to ensure seamless, patient-centered care.
Use electronic health records (EHR) to document clinical encounters and track patient outcomes.
Engage in quality improvement initiatives and contribute to continuous care enhancement.
Educate patients and families on disease prevention, treatment plans, and healthy lifestyle choices.
