About The Position

The organization is seeking a Primary Care Physician to join its growing team in Montgomery following a physician transition. This role offers the time, support, and resources to make a meaningful impact on patients’ lives. The care model features smaller patient panels, interdisciplinary collaboration, and proactive, wellness-focused care for Medicare-eligible adults and underserved senior communities. Physicians build lasting relationships with patients and are supported by an experienced clinical and operational team focused on improving outcomes, reducing barriers to care, and strengthening communities. The practice uses a value-based care model centered on prevention, wellness, and coordinated care. It offers a modern, technology-enabled environment, on-site diagnostics, and support from an interdisciplinary team and dedicated care coordination and population health staff. Ongoing professional development and training in value-based care are provided.
Requirements
- MD or DO from an accredited medical school.
- Board-certified or board-eligible in Internal Medicine or Family Medicine.
- Active or eligible Alabama medical license.

Responsibilities
- Provide primary care to Medicare-eligible adults and underserved senior communities.

- Contribute to improving patient outcomes, reducing barriers to care, and strengthening the communities served.
Benefits
- Base salary ranging from $250,000 for new graduates or residents up to $295,000 for experienced physicians.
- Annual incentive bonus with achievable performance goals; recent physician payouts ranged from $45,000 to $85,000.
