About The Position

Working across two harm reduction program locations, the Prevention Navigator is responsible for engaging participants in comprehensive prevention, harm reduction, and supportive services. As a member of the multidisciplinary team, the Prevention Navigator provides outreach, screening, care coordination, health education, and navigation services to individuals at risk for or living with substance use disorders, HIV, viral hepatitis, and other health conditions. The Prevention Navigator works closely with program staff, medical providers, case managers, behavioral health professionals, and community partners to ensure participants are connected to prevention services, treatment, healthcare, benefits, and other supportive resources. The position promotes low-threshold, participant-centered, and trauma-informed engagement while supporting continuity of care across both program locations.

Requirements

  • Bachelor’s degree in a health field or equivalent in experience preferred
  • Minimum 1 year of relevant non-profit employment experience required (i.e. outreach, testing, group facilitation, case management, etc)
  • Knowledge of HIV/AIDS, homelessness, substance use and mental illness, and LGBT youth issues
  • Good problem-solving skills and ability to work autonomously
  • Strong documentation/computer skills (Word, Excel, etc)
  • Good verbal and written communications skills

Responsibilities

  • Provide direct participant engagement, navigation, and support services across two harm reduction program sites.
  • Conduct participant screenings and assessments related to prevention services, health needs, and social determinants of health.
  • Educate participants on harm reduction strategies, overdose prevention, safer substance use practices, HIV prevention, STI prevention, viral hepatitis, and other wellness topics.
  • Facilitate referrals and linkage to services including primary care, substance use treatment, mental health services, housing, benefits, HIV care, Hepatitis C treatment, and other community-based resources.
  • Support participant enrollment and retention in prevention and treatment programs.
  • Assist individuals in accessing and adhering to HIV prevention services, including PrEP and PEP, as applicable.
  • Promote access to Hepatitis C screening, treatment, and follow-up care.
  • Maintain ongoing communication with participants to encourage engagement and continuity of care.
  • Collaborate with multidisciplinary staff to develop and implement participant-centered service plans.
  • Conduct follow-up activities to monitor referral outcomes and participant progress.
  • Document participant encounters, referrals, and services provided in electronic health records and/or data management systems in a timely and accurate manner.
  • Participate in case conferences, team meetings, quality improvement activities, and program trainings.
  • Maintain confidentiality and comply with all applicable organizational, regulatory, and funding requirements.
  • Support program outreach and community engagement activities as needed.
  • Travel between assigned program locations and community sites as required.
