Requirements

• Graduate of an accredited Medical or Osteopathic School
• Completion of an accredited ABMS-approved family medicine or internal medicine residency
• Ability to obtain a Minnesota Medical Doctor or Doctor of Osteopathy license
• Valid DEA Registration

Responsibilities

• Provide primary care as a Family Medicine or Internal Medicine Physician.
• Collaborate with MTM Pharmacists, Care Coordination, Nurse Call Center, Diabetes Educators, Community Paramedics, and Social Workers.
• Participate in outpatient only call with nurse triage as the first point of contact.
• Utilize Epic electronic health record system.
