About The Position

Elara Caring is seeking a Physical Therapist to provide home-based patient care. The role contributes to the company’s mission of delivering high-quality care in patients’ preferred home settings. The Physical Therapist assesses patients’ functional needs and outcomes, develops and adjusts plans of care, delivers therapy within defined standards, and supports staff and the community through direction and leadership. The role includes travel within the branch or office coverage area, potentially irregular and on-call hours, and physically demanding work.

Requirements

Graduate of an American Physical Therapy Association (APTA)-approved physical therapy program.
Current, unrestricted PT license valid for the state of work.
At least one year of experience in a healthcare facility.
Able and willing to travel within the branch or office coverage area, including travel of up to 50%.
Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.

Responsibilities

Promote Elara Caring’s philosophy, mission statement, and administrative policies to support quality of care.
Provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists.
Review patients’ functional needs and adjust care plans to support optimal outcomes.
Assess patients and design and implement plans of care, incorporating participation by significant others when appropriate.
Conduct therapy practice within defined standards of care.
Direct support staff to help achieve patients’ functional goals.
Document evaluations, treatment goals, and plans; update documentation regularly and maintain prescriptions and signatures.
Provide direction and leadership to staff and act as a resource to promote physical therapy in the community.
