About The Position

InHome Therapy is hiring a licensed home care Physical Therapist to provide services to adult and geriatric patients in their homes in and around Park Ridge. The role offers autonomy over scheduling, a consistent caseload through agency partner networks, and clinical and administrative support. Compensation is paid per visit, with evaluation visits ranging from $65 to $85. Final compensation depends on experience, qualifications, and internal equity. Benefits eligibility varies according to employment status and weekly hours worked.

Requirements

• Active physical therapy (PT) license in good standing.
• BLS CPR certification.
• Reliable transportation to visit patients in their homes.
• Basic comfort using technology, including email, texting, and EMR systems; training is provided.

Responsibilities

• Deliver in-home physical therapy services to adult and geriatric patients in the community.
• Evaluate patients’ mobility, strength, and function and develop individualized treatment goals.
• Implement rehabilitation and therapeutic exercise programs.
• Educate patients and families about recovery plans and preventive care.
• Maintain accurate and timely home health care documentation in accordance with Medicare and agency standards.
