About The Position

InHome Therapy is hiring a licensed home care Physical Therapist to serve adult and geriatric patients in their homes in and around Oak Park. The role offers schedule flexibility, autonomy, a consistent caseload, and clinical and administrative support. The position is paid on a per-visit basis, with evaluation visits ranging from $65 to $85. Final compensation depends on experience, qualifications, and internal equity. A sign-on bonus is available.

Requirements

Active physical therapy (PT) license in good standing.
BLS CPR certification.
Reliable transportation to visit patients in their homes.
Basic comfort with technology, including email, texting, and EMR systems; training is provided.

Responsibilities

Deliver in-home physical therapy services to adult and geriatric patients in the community.
Evaluate patient mobility, strength, and function and develop individualized treatment goals.
Implement rehabilitation and therapeutic exercise programs.
Educate patients and families about recovery plans and preventive care.
Maintain accurate and timely home health care documentation according to Medicare and agency standards.
